嘉定甲申之歲，祖師許真君降于金陵，示陳忠孝之教，溥化眾生，咸歸正道。有王居士者，感蒙聖教，罄捨家貲創崇真道院于天慶觀之西，奉安香火，迄今廟貌存焉。而岑奉師旨共闡玄風，忽值弟子邢道堅、梁道寧亦建勇悟道院於嘉會酒樓之北，為接待仙賓之地。霧輳雲輯者，眾何啻千餘。驗其抱金石之志懷饑渴之心者，惟汪道沖、宋道昇，趙道泰、趙道節、林守一、賈守澄、劉道益、孔守善而已。此數人者嘗讀《西山傳記》稱頌祖師功德有日，于玆邇來宋道昇捧所錄《十二真君傳》至，乞加訂正。因以觀之，見其詞理重復篇章混雜，使覽者易生厭倦深竊惜焉。岑乃校正事蹟分別章句，祈為八十五化，化各著詩。又得邢道堅執卷待旨始終如一，殆可嘉賞，岑愈喜之。詩成，有朱守中來就命參詳，庶無亥豕之訛烏焉之舛，縣是祖師傳王而無可疑議者矣。然則祖師之聖功道行至此愈若青天白日之昭明乎，既修飾道院編集詩傳，復得邢道堅募金鏤梓以衍其傳，綿千載而罔極，豈斯傳之期永哉，使邢、梁二士之嘉名同不泯爾。西山勇悟真人施岑跋。
